DG101- DLAM Guideline Medical records retention -USDA Covered Species
Medical records include, but are not limited to:

Species, weight, gender, identification, PI, and IACUC protocol number

Individual Clinical Animal Health Record form

Lab results and surgical/anesthesia

Procedural and post-op records
-
Vaccination records
-
Any documentation provided by the vendor

Behavioral notes
Medical records:
-
For live animals are transferred to an active IACUC protocol when the protocol closes or expires.

Are transported from temporary record locations to DLAM’s central record storage, located in CHS 1st floor Vet Suite.

Are destroyed three years following the IACUC protocol closure or renewal date when animals are no longer alive.
